Brown Geriatric Evaluation Clinic

Opened in 1998, the Drs. Charlotte & David Brown Geriatric Evaluation Clinic is a comprehensive evaluation and management program that provides specialized services for older adults and their families, as well as other professionals.

Our outpatient program addresses common, but often complex medical issues in older adults – especially memory loss and dementia. Our expert team provides patients and families with individualized needs assessments, baseline evaluations and ongoing management to help optimize everyone's quality of life.

What is a Geriatric Assessment?

The purpose of a geriatric evaluation is to review an older adult's cognitive, psychological, social, medical and functional status. Our assessments are conducted by a neuropsychologist, board-certified geriatrician, or an advanced practice nurse who specializes in geriatrics.

The goal of an evaluation is to help the patient and their family to make appropriate short- and long-term healthcare decisions. Professional recommendations by our team of experts can assist in maximizing safety, functions of daily living, and quality of life. Ultimately, these recommendations may enable individuals to remain safely at home for as long as possible.

Who Benefits from a Geriatric Assessment?

Those who benefit most from a geriatric evaluation are older adults with specific concerns including memory, mobility, repeated falls, medication management, depression or chronic health ailments, as well as those who are unable to make safe decisions in their current living environment.

When changes take place in a senior’s functional capabilities, or if concerns arise regarding one's ability to manage independently, an evaluation is a positive way to help identify and address the issues at hand.

Patients, families and caregivers can use the information gained through a geriatric evaluation to establish a baseline, and to make informed decisions about community resources, medical services and residential options.

After the Evaluation

Following the evaluation, a conference is held with the patient, family and caregivers to discuss observations and recommendations. Family members and others are encouraged to provide their input throughout the post-evaluation conference process.

A written assessment will include recommendations for care that may be shared with the patient’s primary care physician. Optional home care services, care management, and follow-up monitoring are always available.

Role of Primary Care Physician

The Brown Geriatric Evaluation Clinic does not replace the need for a primary care physician. Rather, the Clinic will collaborate with physicians and other professionals to best address patient and family care needs.

Available to Everyone

The Brown Geriatric Evaluation Clinic serves individuals from all areas and can accommodate evaluations for older adults living at home, in independent living residences, assisted living settings, lifecare facilities or nursing homes.

The cost of the program is partially covered by Medicare and the balance is the patient’s responsibility.
